Weight regain and reflux after banded RY Gastric bypass, with intra-luminal erosion of banding, endoscopic removal of band, reposition of the herniated pouch, cruroplasty, plication of the dilated pouch and GJ anastomosis, modified Nissen and distalization according to Sugarman.
